Kids/Youth Ministry Volunteer Application

This application is to be completed by all applicants for any position (volunteer or compensated) involving the supervision or custody of minors. It is being used to help the church provide a safe and secure environment for those children and youth who participate in our programs and use our facilities. Please know that the following information will be kept secure and confidential- shared only with the appropriate pastoral and professional staff as we deem necessary. We do run a background check on all of our volunteers over the age of 18.
